From a QCAD user:

How can I add items, edit items or delete items in the QCAD library browser?

The items in the part library of QCAD are normal, regular drawing files in DXF or DWG format. These files are located in the 'libraries' folder of your QCAD installation. You can edit, delete or add new files to the libraries folder as to any folder.

Adding items using QCAD:
You can add new items to the library, by saving additional files into the 'libraries' folder.
If you prefer, you can also add items to the library by making a selection in your drawing and using the tool 'Block - Create Library Item'.

Editing items using QCAD:
You can edit library items by editing the files in your libraries folder.
If you prefer, you can edit items directly from within QCAD by right-clicking on an item and choosing 'Edit...'. Save your drawing when you are done.

Updating the library browser contents
After changing or deleting a library item, be sure to update the library database by clicking the button in the bottom right corner of the library browser.

Notes for Mac OS X users:
- Please note that under Mac OS X, that libraries folder is located inside the application bundle.
- To access the libraries folder, locate the QCAD-Pro.app application bundle using Finder
- Right-click on the application bundle and choose 'Show package contents' from the context menu
- Navigate to 'Contents/Resources/libraries/templates/metric'
